Job Description Summary ‎ Taking primary care to the leading edge of medicine through an innovative and interconnected value-based platform from which today’s physicians rise to tomorrow’s healthcare challenges. ‎ How will you make an impact & Requirements ‎ Clinical Care Perform comprehensive patient evaluations, including history, physical examination, and diagnostic interpretation. Diagnose and manage acute, chronic, and complex medical conditions. Develop individualized treatment plans and monitor patient progress. Order, interpret, and follow up on diagnostic tests (labs, imaging, PFTs, cognitive screenings, cardiac assessments, etc.). Provide preventive care, including immunizations, screenings, and lifestyle counseling. Manage medication therapy, including prescribing, monitoring, and reconciliation. Participate in care gap closure, chronic disease management, and preventive health initiatives. Utilize evidence‑based guidelines to improve patient outcomes and reduce avoidable hospitalizations. Document care accurately to support quality reporting, risk adjustment, and compliance. Engage in population health strategies, including outreach, follow‑up, and care coordination. ‎ Compensation Range: $172,938.00 to $259,407.00 The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting.
